This form constitutes notification of withdrawal. It is dated and time-stamped on submission, and a copy is emailed to you: keep it, as it evidences your request.

Withdrawing does not exempt you from sending the items back: once we receive your request, we will send you the return instructions. Return postage remains payable by you, except for a faulty item or an error on our part.
